New Patient Visit
Start your journey with FWP through a comprehensive initial visit. During this visit, we’ll review your medical history, allergies, and current medications. Together, we will create customized preparedness/travel pack prescriptions tailored to your unique needs. Please note that medications are paid for separately to the pharmacy.

Follow-Up and Sick Visits (FOR ESTABLISHED PATIENTS)
For the next 12 months, enjoy follow-up and sick visit consultations at a discounted rate. This service ensures continuous and convenient access to healthcare whenever you need it.

- Ongoing, consistent care throughout the year

Follow up asynchronous email/text visits (based on complexity)
For quick consultations, whether you need new prescriptions or just have minor health concerns, our brief text or email visits provide an efficient and convenient solution.

- Ideal for minor issues or follow-ups

House Calls
Experience the convenience of personalized in-home visits. Available in Alachua, High Springs, and Fort White areas, this service brings comprehensive care to the comfort of your home.

- Available within specified local areas
$150-$300 DEPENDING ON DISTANCE
Non-business hours PATIENT VISITS
Please note that for house call services requested after 5:00 PM on weekdays, as well as on weekends and holidays, an additional fee of $75 will be applied to the standard house call rate.

cancellations made less than 24 hours in advance will incur a $75 fee.
To help us serve all our patients effectively, we kindly ask that you notify us of any cancellations or rescheduling at least 24 hours before your scheduled appointment. This policy enables us to offer timely care to other patients in need.
Monthly Deprescribing Support
Some patients undergoing deprescribing, especially for SSRI medications, will require closer monitoring.
This involves frequent text and/or email check-ins throughout the weaning process.
PHASE 1
After your initial visit to establish care, we will discuss your individual needs and create a personalized plan.
PHASE 2
For these patients, we offer a monthly membership for $100. This fee covers all non-emergency text and email communications within the month.
PHASE 3
Additionally, any necessary phone or telehealth visits during this period are available at a reduced rate of $50 each.
